Homedica is proud to participate in the CMS GUIDE Model (Guiding an Improved Dementia Experience), a national program designed to improve care and support for individuals living with dementia and their caregivers. Our program offers personalized services that enhance quality of life, provide expert care planning, and ease the challenges of managing dementia at home.

Supporting Your Loved One with Dementia
Homedica’s GUIDE Program is designed to help families navigate the challenges of dementia care with confidence and support. Through personalized services and a dedicated care navigator, we partner with you to make caring for your loved one easier, safer, and less overwhelming.

Who Can Enroll in the Program?
The GUIDE Program is available to individuals living with Alzheimer’s disease or other forms of dementia. Patients must have a formal dementia diagnosis and need regular support with daily activities or care needs. Caregivers who provide day-to-day assistance to their loved one are also included in the program and can benefit from added support and resources. If you’re unsure about eligibility, our team can help guide you through the process.

Planning Care Together
We work with you to create a personalized care plan that reflects your loved one’s needs and goals. This plan helps guide medical decisions, addresses safety concerns, and ensures you have a clear path forward as dementia progresses.

Managing Symptoms and Behaviors
Our care team helps ease symptoms such as memory loss, confusion, sleep problems, or agitation. We offer guidance, medication adjustments, and practical strategies to improve comfort and reduce stress for both patients and caregivers.

Education and Guidance for Families
We provide hands-on education to help you understand dementia, manage day-to-day challenges, and build confidence in caregiving. You’ll receive tips and resources tailored to your situation to make caregiving feel more manageable.

Your Dedicated Care Navigator
Every family in the GUIDE Program is connected to a dedicated care navigator. This person serves as your main point of contact, helping coordinate care, answer questions, and guide you to resources and services when you need them most.

24/7 Access to Support
You’ll have round-the-clock access to our team for any questions or concerns that arise. Whether it’s a new symptom or an urgent issue, we’re here to offer support whenever you need it.

Coordinating with Your Doctors
We help coordinate care between your loved one’s primary care doctor, specialists, and other providers. Our team ensures everyone is on the same page, making it easier to avoid hospital visits and keep care consistent.

Accessing Community Resources
We help connect you to local support groups, respite services, and community organizations that can offer extra help along the way. Our care navigator can assist you in finding resources that are right for your family.

Free Respite Services Covered by Medicare
Through the GUIDE Program, eligible caregivers have access to free respite care services paid for by Medicare. These services provide short-term relief, giving you time to rest, recharge, or attend to other needs, knowing your loved one is safely cared for.
