Homedica House Calls, a division of The Carpenter Health Network, is a trusted leader in innovative, patient-centered primary care. Serving patients across Louisiana and Mississippi, we provide high-quality care through in-home visits, telehealth, and post-acute facility medicine.
Our multidisciplinary team—including physicians, nurse practitioners, care navigators, medical assistants, and support staff—deliver personalized, comprehensive care aimed at improving health outcomes, enhancing quality of life, and reducing the need for traditional office visits.
In addition to primary care, Homedica offers several specialized programs designed to meet the needs of complex and aging populations. These include Advanced Supportive Care (ASC), our palliative care program for patients facing serious illness, Chronic Care Management (CCM) for those with multiple chronic conditions, and behavioral health services provided by board-certified psychiatric nurse practitioners. We also participate in the CMS GUIDE model, which supports individuals living with dementia and their caregivers through free respite services, structured care planning, and support from a dedicated GUIDE care navigator.
With a strong focus on comfort, continuity, and collaboration, Homedica partners closely with home health agencies, hospice providers, assisted living communities, skilled nursing facilities, and other post-acute facilities—ensuring patients receive coordinated, compassionate care wherever they call home.
